  I hereby, in good faith, release you, the adult chaperons, and Mater Dolorosa and St. Dominic Churches from all liability for any accidental injuries or harm or loss sustained by my child while participating in this activity.

I understand that this event will take place away from the Church grounds and that my child will be under the supervision of the designated supervisor(s) on the stated date(s). I further consent to the conditions stated above on participation in this event including the method of transportation.
